Questionnaire for the diagnosis of acute allergic reactions

Dear parents!

Scientists at our Institute are developing a method for diagnosing acute allergic reactions in children. These studies require detailed information about the prevalence and pattern of such reactions. Therefore, we are conducting a survey of parents with children aged 0 to 17 years.

Please answer a number of questions that will help identify the presence of health and life-threatening conditions in your child that require additional monitoring and examination. Based on the results of the survey, you will be offered a consultation with an allergist and, if necessary, an examination. If you have several children, you can fill out this form for each child.
